Y Medical Associates, Inc. (YMA) employs dedicated professionals to handle our referral management department. We can assist in the verification of coverage for home infusion therapy and obtain the necessary pre-certification or authorizations for our services.
The YMA intake team is experienced in transitioning members from other providers; we offer a streamlined referral process and seamless transition. YMA can accept most insurance but in those circumstances where we can’t, we will help to find a suitable provider for your care needs.

To refer a patient to YMA for infusion therapy services, you will need to provide the following patient information:
- Name
- Address
- Phone number
- Social security number
- Date of birth
- Health insurance card(s) or name of insurance carrier
- Health insurance member ID number
- Primary Diagnosis
- Allergies
- Height and Weight
- Current vascular access device used and any associated placement reports
- Prescribing Physician Name and associated orders
- Name of Physician following the patient during our care
- Past clinical notes or letter of medical necessity
By providing us with any additional pertinent history or letters of medical necessity the referral process can be more efficiently expedited. Once all of the required information is received, YMA will verify the patient’s benefits and obtain any necessary preauthorization to admit the patient to our services.
